The role of the community matron in supporting patients with long-term conditions through holistic case management is well established, as is learning through critical reflection and using case studies for continuing professional development.
This article draws on a case study to provide a critical analysis of the community matron service, which offers intervention and case management for a patient who had been diagnosed with heart failure.
