Research by the Queen’s Nursing Institute has revealed that unsafe hospital discharge is one of the biggest risks facing homeless people. With their needs unmet in hospital, they are discharged with nowhere to go but a hostel. Holistic care by multidisciplinary services can transform their health and help them out of homelessness.
