A radical realignment of health and care services is bringing hospital-level care to people in their own homes, reports Erin Dean

Health and social care organisations in Southwark and Lambeth are joining forces for an ambitious programme in which nurses work with other professionals to deliver hospital-level care to patients in their own homes. Early indications are that the integrated approach is reducing admissions and improving care.
