Improving discharge planning standards

This study shows how nursing staff on a busy surgical ward improved their system of patient discharge using the Dynamic Standard Setting System (DySSSy). Initial audit indicated the standard was not being met, but after consideration of the findings and a re-evaluation of practice, a subsequent audit showed considerable improvements.
