A resource to support the High Impact Change Model for Managing Transfers of Care has been developed by the Local Government Association and Association of Directors of Adult Social Services. The online resource provides evidence-based examples of work for the eight system changes that have been identified as having greatest effect on, for example, reducing delayed discharge, early discharge planning, monitoring patient flow, and multidisciplinary or multi-agency discharge teams. It also includes examples of emerging practice, and links to further guidance and information.
