ESCAPE DIABETES Act Now (EDAN) is a one year pilot scheme that has recently launched in Hartlepool and aims to reduce the number of local people developing diabetes. It takes a preventative approach by working with at-risk, pre-diabetic adults, referred by their GP or practice nurse, to help improve their lifestyles.
