Caring for the dying is one of the most difficult parts of nursing. But a district nursing team serving a population of older people in rural Suffolk has found a way of enabling patients to have the care they want in the place of their choosing. The Gold Standards Framework has helped the team make it possible for more than four out of five patients to die at home.
