Ty Hafan: ‘Here to hold your hand’

Community, specialist and hospital nurses play a key role in referring children and families for much needed hospice services. The number of referrals from the healthcare profession to Ty Hafan, the Children’s Hospice in Wales, has been low this year which has prompted the launch of an information campaign: ‘Here to hold your hand’. The aim is to raise awareness about the referral process and, in turn, increase numbers of families being referred to Ty Hafan. This report explains the rationale and background to the campaign and the reasons why nursing professionals should advocate use of the service to families of children with life-limiting diseases.
